Good coaching templates do two things. They give your client a structured approach so every session moves them forward, and they give you a coaching practice that runs on systems instead of memory. This page collects 15 free printable life coaching templates in one PDF pack: a client intake form, a coaching agreement, the wheel of life, a first session plan, session notes, goal setting and action step worksheets, trackers, reflection prompts and an end of program review. Each one is explained below with who it is for and how to use it in a session, and you can get the whole pack free by email using the box below.

Why coaches need templates

New coaches often feel that worksheets make coaching less personal. The opposite is true. A template holds the structure so you can give your full attention to the client in front of you. It makes sure you ask the same important questions of every new client, that you track progress rather than guess at it, and that your client leaves every coaching session with clear action steps instead of a nice conversation. Templates also protect you. A signed coaching agreement with clear boundaries, fees and cancellation terms is the difference between a hobby and a professional coaching business. Below is the set I use and teach in the Soul Coach Certification, adapted so that any life coach can use them.

The 15 free printable life coaching templates

1. Client intake form

For: every new client before the first session. Use it: send it with the booking confirmation and read it before you meet. It collects contact details, the client’s goals, what they have tried before, what support they have, any health or mental health context they want you to know, and how they prefer to communicate. It also includes a short consent statement that coaching is not therapy or medical care.

2. Coaching agreement

For: every paid client. Use it: sign before the first paid session. The agreement template covers the scope of coaching, the number and length of sessions, fees and payment terms, cancellation and rescheduling policy, confidentiality, and the responsibilities of both coach and client. Have a lawyer in your state check it before you rely on it.

3. Wheel of life

For: the first session and every three months after. Use it: the client scores eight different areas of their life from 1 to 10, career, money, health, relationships, family, personal growth, fun and environment, then shades the wheel. The shape shows at a glance where the energy is going and where it is missing, and it gives you the first coaching conversation: which area, if it moved by two points, would change everything else?

4. First session plan

For: the coach. Use it: a one page agenda for the first session: welcome and agreement check, the client’s story in their words, wheel of life, the one big goal, what success looks like in three months, and the first action step. It keeps a nervous new coach on track and gives the client a clear, confident start.

5. Session notes

For: every session. Use it: record the purpose of the session, the client’s current situation, key insights, decisions, action steps agreed and the focus for the next session. Keep notes brief and factual, store them securely and review them for five minutes before the next session so the client never has to repeat themselves.

6. Goal setting worksheet

For: the client, in session two or three. Use it: turns the big goal into a specific, measurable, dated outcome, then asks why it matters, what will be different when it is done, what could get in the way and who can help. The why question is the one most clients skip and the one that keeps them going in week six.

7. Action steps tracker

For: the client between sessions. Use it: three to five action steps per session, each with a deadline and a box to tick. The client brings it to the next session. Done, not done and why becomes the opening of every coaching conversation and it builds accountability without nagging.

8. Weekly agenda

For: clients in a weekly or fortnightly coaching program. Use it: a simple weekly planner that puts the action steps into real days, with space for one priority, three tasks and one act of self care. Most goals fail at the calendar, not at the vision.

9. Progress tracker

For: the coach and the client, monthly. Use it: a grid that tracks the main goal and three supporting habits week by week over twelve weeks. It makes the client’s progress visible, which matters most in the middle of a program when motivation dips and the finish line is not yet in sight.

10. Reflection prompts

For: homework between sessions. Use it: twelve journaling prompts, one per week, such as what did I avoid this week and what was I protecting, and where did I feel most like myself. Reflection turns experience into insight and gives quieter clients something to bring to the session.

11. Values worksheet

For: clients who feel stuck or torn between options. Use it: the client chooses their top ten values from a list of sixty, narrows them to five, then scores how well their current life honors each. Decisions get easier when the values are on paper.

12. Career change worksheet

For: career coaching clients. Use it: maps what the client wants to keep, lose and gain, their transferable skills, three possible directions and one low risk experiment for each. It stops the all or nothing thinking that keeps people in jobs they have outgrown.

13. Boundaries worksheet

For: clients dealing with burnout, people pleasing or difficult relationships. Use it: identifies where energy leaks, what the client needs instead, the exact sentence they will say, and what they will do if the boundary is not respected. Scripts matter. Clients can rarely improvise a boundary in the moment.

14. Coaching program outline

For: the coach designing a package. Use it: a one page plan for a six or twelve session program: the promise, who it is for, the theme of each session, the templates used in each, and the result at the end. It turns one off sessions into a program you can price, sell and deliver consistently.

15. End of program review

For: the final session. Use it: the client re-scores the wheel of life, compares it with the first, lists what changed, what they learned about themselves and what they will keep doing. It closes the coaching relationship on a positive note, gives you a testimonial in the client’s own words and opens the conversation about what comes next.

How to use the templates in a coaching program

A simple twelve week program uses them in this order. Before session one, intake form and coaching agreement. Session one, first session plan and wheel of life. Session two, values worksheet and goal setting worksheet. Every session, session notes for you and the action steps tracker for the client, with the weekly agenda if they need help with time. Monthly, the progress tracker. Weekly homework, the reflection prompts. Sessions six to ten, the career change or boundaries worksheets as needed. Session twelve, the end of program review. The program outline holds the whole thing on one page so you can sell it before you deliver it.

Print the pack once, keep a master copy, and photocopy or print the pages you need per client. If you coach online, share the PDF and have clients fill it in on a tablet or send you a photo of the completed page before the session.

A note on scope. These templates support coaching, which is forward looking work with clients who are functioning well. The intake form asks clients to share mental health context so you can refer them to a therapist or doctor when their needs go beyond coaching. Clear boundaries about what you do and do not do protect both of you.

Frequently asked questions

What templates does a life coach need?

At minimum a client intake form, a coaching agreement, a session notes template, a goal setting worksheet and an action steps tracker. The wheel of life, a progress tracker and an end of program review turn individual sessions into a structured program.

What is the wheel of life template?

The wheel of life is a coaching tool where a client scores eight areas of their life from 1 to 10 and shades a circle divided into segments. The shape shows which areas are thriving and which need attention, and it is usually used in the first session and repeated every few months.

Are these life coaching templates really free?

Yes. The 15 template pack is a free PDF download. You can print it for your own clients as often as you like. Please do not resell the templates.

Yes. The PDF can be filled in on a tablet or printed, photographed and sent back. Many coaches share the relevant page before each session and review it together on screen.

Do I need a coaching agreement for every client?

Yes, for every paid client. It sets the scope, fees, cancellation policy and confidentiality, and it protects both you and the client. Have a lawyer in your state review the template before you use it.

How do coaches track client progress?

With session notes for the coach, an action steps tracker the client completes between sessions, a monthly progress tracker for the main goal and habits, and a repeat of the wheel of life every three months to compare scores.
